WHAT DOES NCQA STATUS MEAN FOR YOU?
Berkeley Family Practice is proud to share with our patients that we were awarded NCQA’s Patient-Centered Medical Home status in May 2015. The patient-centered medical home is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams.  ​Research shows that PCMHs improve quality and the patient experience, and increase staff satisfaction—while reducing health care costs. Practices that earn recognition have made a commitment to continuous quality improvement and a patient-centered approach to care.

In addition, these medical homes are driving some of the most important reforms in health care delivery today. A growing body of scientific evidence shows that PCMHs are saving money by reducing hospital and emergency department visits, reducing health disparities, and improving patient outcomes.


This program highlights primary care providers who have proven they can coordinate care among all of a patient’s doctors and other health care providers.  As a patient, you receive better understanding of treatment options and participate in decisions about your health care.  You have better access through expanded hours and the web.  Providers work in teams to prevent problems and manage chronic conditions to keep you healthy.  The result is better quality and fewer costly trips to emergency rooms and hospitals. 


NCQA Patient-Centered Medical Home standards show primary care providers how to make these important changes. We offer three levels of Patient-Centered Medical Home recognition.  Level 3 is the highest level. 

The evidence we present here outlines how the medical home inspires quality in care, cultivates more engaging patient relationships, and captures savings through expanded access and delivery options that align patient preferences with payer and provider capabilities.
